Millicent’s questline follows a young woman with scarlet rot from a ruined church in Caelid to the Haligtree, where you face the questline’s big choice: help her or betray her. It rewards two of the best talismans in the game for Dexterity builds.
| Starts | Church of the Plague, Caelid |
| Ends | Elphael, Brace of the Haligtree |
| Summons | Magma Wyrm Makar, Godskin Apostle, Black Blade Kindred |
| Main rewards | Rotten Winged Sword Insignia or Millicent’s Prosthesis, Miquella’s Needle |
Millicent Questline walkthrough
- Meet Millicent. Find her in the Church of the Plague, north-east of Gowry’s Shack in Caelid. She is ill with scarlet rot.
- Talk to Gowry. At Gowry’s Shack, Gowry asks you to bring the Unalloyed Gold Needle.
- Defeat Commander O’Neil. He guards the Swamp of Aeonia in Caelid and drops the Unalloyed Gold Needle.
- Repair the needle. Give it to Gowry, leave the area and come back to collect the repaired needle.
- Cure Millicent. Give her the needle at the church. You get the Prosthesis-Wearer Heirloom and she moves on.
- Optional summon: Makar. Her summon sign appears for Magma Wyrm Makar in the Ruin-Strewn Precipice.
- Find her on the Altus Plateau. She stands near a cliff north of the Erdtree-Gazing Hill grace and asks about her lost arm.
- Get Valkyrie’s Prosthesis. It is in a chest on the north-west edge of the Shaded Castle, guarded by a Cleanrot Knight. Give it to Millicent.
- Godskin Apostle. Summon her for the Godskin Apostle in Windmill Village, then rest at the Windmill Heights grace and talk to her.
- Optional summon: Black Blade Kindred. Her sign appears for the Black Blade Kindred in the Forbidden Lands.
- Mountaintops of the Giants. Talk to her at the Ancient Snow Valley Ruins grace.
- Elphael. After the Putrid Tree Spirit, meet her at the Prayer Room grace in Elphael. Two signs now appear near the lake.
- The choice. Use the golden sign to help her fight her four sisters, or the red sign to invade and kill her.
Ending and rewards
Help her: you get the Rotten Winged Sword Insignia. Reload the area to find the Unalloyed Gold Needle where she sat; use it on the flower in Malenia’s arena after beating her for Miquella’s Needle and a Somber Ancient Dragon Smithing Stone (see all Ancient Dragon Smithing Stone locations).
Betray her: you get Millicent’s Prosthesis. Both talismans boost attack during combos and are core to the Dexterity Katana build.
Tips
- Help her if you want Miquella’s Needle: it can undo the Frenzied Flame ending.
- After the quest you can talk to Gowry one last time.
- Track the bosses you beat with the boss checklist.
